For Part 2 of this Assignment, you will write an essay to summarize your success or challenges with your target behavior change project. Please note that you will not be graded on how successful you were with changing the target behavior. You will be graded on
the thoroughness of your insight and reflection on the project
the level of critical thinking demonstrated in your application of one of the health behavior
change theories/models
the level of support you provide in your arguments and statements by citing reliable and relevant sources
your use of concepts and language on behavior change discussed in the readings and class-the clarity and quality of your writing

You should choose one of the behavior-change theories or models discussed during Week2 on Meeting the Challenges. You may choose one of the following theories or models as a framework to interpret your success or difficulty with changing your target behavior:The Transtheoretical Model (also called the Stages of Change Model, discussed in Chapter 1 of the Core Concepts in Health Textbook), the Health Belief Model, or the Theory of Planned Behavior (introduced in Chapter 2 and covered in more detail in Chapter 8 of the AFMC Primer on Population Health). You will apply the model or theory in the Results and Critical Analysis Section.

Your Essay should be organized into the following sections and you should cite reliable sources throughout. You should use headings (e.g. Introduction, Methods, Results and Critical Analysis, Conclusion) and subheadings (e.g. Applying the Health Belief Model) to organize your essay. Note that you are not allowed to cut and paste your answers from Part 1 of the Assignment. You should summarize and expand on what you said for Part 1 as well as present new information:
Introduction (5 marks)
Describe the overall goal and what the focus of your target behavior was. Explain your motivation for choosing this target behavior as well as any relevant past experience with this behavior. Describe both the adverse health effects of not changing the behavior as well as the health benefits of changing the target behavior. Be sure to differentiate between short-term and long-term effects.
Methods for changing the target behavior (5 marks)
Describe the plan you used to change the behavior. Include a description of the specific goal and subgoals, the time frame for meeting each goal, the strategies you used, how you tracked your behavior, what kind of information you recorded, any modifications to your environment, and rewards or other reinforcements you used to try to help you to reach your sub-goals and the final goal.
Results and Critical Analysis (20 marks)
Review your health journal entries or other data and report on your level of success or failure with changing the target behavior. Be sure to give specific numbers to quantify your level of success or failure.
What patterns emerged in your target behavior over the course of the project (expand on those you noted for Part 1 of the assignment and note new patterns)? What factors were most relevant in your success or what factors contributed to challenges with changing the target behavior? Be sure to integrate concepts from reliable sources on successful behavior change, such as Chapter 1 of the textbook.
Describe how you dealt with challenges and whether you modified your plan during the course of the project. If you were not able to successfully deal with the challenges, reflect on why that was the case.
Discuss 1 of the behavior change models or theories introduced during Week 2 of this course (e.g. the transtheoretical/stages of change model, the health belief model, or the theory of planned behavior). Describe the model/theory and apply it to your behavior change project. What aspects of the model/theory are relevant to understanding your successes or difficulties with changing the target behavior?
4. Conclusion – 5 marks
What, if anything, would you do differently the next time to improve chances of success?
Describe one or two related health behaviors that you would like to change going forward.
How can you apply some of the skills you learned in this project to these other behaviors you would like to change?
What are your general reflections on this project? Was it worthwhile? Did you learn something new? (you won’t be marked down for honesty!)

5. Other requirements
5 marks for Writing Style, Mechanics, and Proper Citing of Sources
The essay was well-written, easy to read, and well-organized into sections.
Errors in spelling, punctuation, sentence structure, or word usage were minimal to none.
All statements that do not belong to the author were supported by a citation in APA format and a full list of references was provided in APA format.
At least 4 sources were cited, and 2 of them were scholarly sources (eg journal articles – do not have to be primary).
The assignment was properly formatted according to the detailed guidelines.

Name and Waterloo ID number
Title of Essay
Date
Word count (for body of essay – not title page or references).

The body of the essay should be 2000 words in total (plus or minus 500 words). Use the Word Count tool in Microsoft Word to keep track. The body does NOT include the title page or references.
Use 12-point font and double line spacing.
Insert page numbers (top or bottom)
You must cite at least 4 different reliable sources (in APA format). 2 of your sources must be scholarly (e.g. journal articles).
Provide a list of references in APA format.
