You are the health information manager at (( Kelburn Recovery Centre MB community care centres ” Mental Health and Addiction Services” )) you and your team have been invited to program committee , at the last meeting, the committee decided to develop a new virtual / remote outreach program before the next meeting , you advised the committee that you and your team would do some research on the topic and present a report , when completing your report you must keep in mind the strategic plan of Kelburn center , population health of the area you serve ( including demographics) and ensure that it follows the principles of capacity building . choose one virtual/remote outreach areas tat may not yet be part of the organization in other words new outreach . this outreach must be able to be delivered virtually or remote . do not use one already use at the organization, use SWOT (3 of each strengths , weakness opportunities and threats) Analysis matrix to assist you and your team in this process, in essence you are to explain who the primary stakeholders are and intended target, what the community outreach program is about , alignment to strategic plan, support of capacity building , demographics ( internal and external ) SWOT assessment appropriate use of graphs and charts. Developed an implementation strategy ( with possible timeline . ensure you give a recommendation on your outreach programs you have given and why you recomment it.
